Staff of Warding

The ultimate in personal elemenstorial protective devices, the Staff of Warding can create a perfectly invulnerable barrier around the wielder with a duration of several hours. However, the inventor Fanastro the Pretty Snazzy was overambitious while creating the artifact, thinking of poisonous vapors, he extended the invulerability to include airtightness. As the Staff does not allow premature cancellation of the protective field (to combat dispellation elemenstation), no one has yet survived using it.

This is widely theorized to be the cause of Fanastro the Pretty Snazzy's death.
